Job Responsibilities : Responsible for providing clinical/case management services to patients and their families in order to promote the optimal individual, interpersonal and social functioning of the patient and assure reintegration into the community or the environment most suited to the patients continued well being. Responsible for supporting patient treatment goals by conducting individual psychotherapy, supportive therapy and group therapy sessions with patients. Conducts initial social service assessments upon patient admission and collaborates with outside providers/referral agencies and family members to develop treatment plan in a timely manner.
Job Requirements : Must have strong interpersonal skills and the ability to work within a multidisciplinary treatment team. Must be able to demonstrate excellent organizational and time management skills in a fast paced setting. Masters degree in Clinical Social Work from an accredited graduate school of Social Work OR Masters degree in counseling or other behavioral health discipline. Must have a valid LICSW or LCMHC in the State of Vermont. Minimum one year social work experience.
